University Fire Department Release of Records

Release of Department Records
All requests for records must be submitted in writing.
Any form of written correspondence (letter, subpoena, e-mail) is sufficient.  The Fire Department does not create or compile custom summaries, lists or reports of fires or other emergency responses.

Once a request is received, it can take up to 14 days to process.

I. Incident Reports

Incident reports within the National Fire Reporting System (NFIRS) is public information and may be released when requested. All incident records are public except when a criminal matter is pending. Persons other than the property owner, renter, state or federal agency will be charged a processing fee for these requests.

Incident Report Processing Fees

If you are not the property owner, renter, state or federal agency, the records processing fee is $40 each report requested. Additional fees to cover supply costs such as USB drives or CD’s is $7.50 (media fee) (if specifically requesting) an electronic incident record on digital medium. Total cost for digital copy on electronic medium is $47.50 or $40 (processing fee) + $7.50 (media fee). Only checks made payable to University Fire Department will be accepted, we do not accept credit cards at this time.

II. EMS Patient Care Reports (PCR’s)

EMS reports or Patient Care Reports (PCR’s) are considered confidential medical records and are protected by privacy laws. PCR’s are available to the patient at no cost and require government issued identification (examples: driver’s license, passport, state id) for release of records while protecting patient privacy. Patients can pick up their reports at Stn.11 Headquarters, 1011 N. Chandalar Drive.
If you are not the patient or next of kin, a processing fee is applied as well as required proper identification to protect patient privacy to release records.

Identification Documentation

Patients can have personal representation but must have proper documentation by meeting one of the following criteria to request a medical record:

1. Request by patient (picture ID required); or
3. Request by an individual possessing a patient's Power of Attorney (picture ID required and notarized copy of Power of Attorney required); or
4. Request by parent or legal guardian if the patient is a minor or incapacitated (picture ID and proof of relationship required); or
4. Request by parent or legal guardian if patient is a minor or incapacitated (picture ID and proof of relationship required); or
5. Request by attorney upon presentation of notarized medical authorization for release from patient; or
6. Presentation of valid subpoena or signed warrant for production of evidence or court order issued by a court of competent jurisdiction; or
7. Unobjected to Request for the Production of Documents in a filed civil action (lawsuit).

III. EMS Billing Inquiries-Outside Agency
University Fire Department is not responsible for emergency service billing. The Fairbanks North Star Borough has contracted with Systems Design EMS.
