- Our processing times begin the day we receive your application at a passport agency or center, not the day you mail your application or apply for a passport at a local acceptance facility.
- We are no longer using door-to-door timeframes to describe routine and expedited service.
- Mail times vary across the country. Plan ahead and apply early.
- Consider the total time it will take to receive your passport when you are booking travel.

First time applicants must complete an available at the ÃÛÌÒÓ°Ïñ Campus Mail Center Passport Services office and at .
Note: Application should be completed prior to submitting.
Note: Youths 16 or 17 years old must apply in person with one parent.
Submit the following documents with the completed Form DS-11:
- Proof of United States Citizenship (only one of the following is required)
Note: All birth records must meet U.S. Department of State guidelines.
- Original State-issued Birth Certificate with raised, colored, or embossed seal
- Certified copy of State-issued birth certificate with raised, colored, or embossed
seal
- Copies of birth certificates are not acceptable. Birth certificates may be obtained at
U.S. Citizens born abroad may submit one of the following:
- Original Certificate of Naturalization (Adults 18 or older)
- Original Form FS 240, Consular Report of Birth Abroad
- Original Form DS-1350, Certification of a Report of Birth Abroad
- Original Certificate of Citizenship
- Applicants may be required to submit the following in addition to proof of citizenship:
Original name change documents (marriage license, divorce decree, adoption papers)
- Original State-issued Birth Certificate with raised, colored, or embossed seal
- Identity Verification
- Current state-issued driver's license Note: Out of state driver’s license requires additional ID
- Current state-issued ID Card
- Current Military ID
Note: If a full adult driver license is not available, multiple forms of ID must be submitted. Please contact our office for another usable ID.
Minors aged 16 or 17 are not required to present photo ID if the accompanying parent has a valid current ID, such as current adult driver license, valid adult passport, or Military ID. If the applicant has a valid ID it should be presented at the time of application.

- Photocopy of ID front and back (Copies can be made at the Passport Services office)
- Passport Photo: One recent passport photograph taken within the last six months Photo
service provided by Passport Services for $15.00.
- Two Required Payments
- 1st payment: to U.S. Department of State (Payable by check or money order only.)
- 2nd payment: to ÃÛÌÒÓ°Ïñ Campus Mail Center (Payable by check, money order, cash, credit / debit).
- 1st payment: to U.S. Department of State (Payable by check or money order only.)
Children under 16 years of age must apply in person with both parents present (for providing consent and proof of parental identity).

Very Important: When presenting a passport to prove citizenship of a child, a birth certificate listing
the names of the parents is also required to verify the parents’ relationship to the
child.
The parent(s) listed on the birth certificate or on the legal document granting custody
or guardianship must be present to sign the passport application in the presence of
the Passport Application Acceptance Clerk.
If one parent cannot be present, a completed and notarized Form DS-3053, Statement
of Consent for A Minor Under Age 16, must be presented and the front and back photocopy
of the non-appearing parent’s photo ID (example - driver license).
Submit the following documents and payments with the completed Form DS-11:
- Original proof of U.S. citizenship: (Photocopies are not acceptable)
- Original or certified copy of a U.S. birth certificate (Birth certificates may be
obtained at )
- Naturalization Certificate
- Consular Report of Birth Abroad
- U.S. Passport that is not damaged.
- Proof of parental legal guardian relationship to a minor applicant must be submitted.

- Examples include:
- Original or certified birth certificate or document showing legal custody or guardianship.
- Documents must be in English or have a certified English translation; photocopies
are not acceptable.

- Photo identification of parent(s): Parents may use valid driver’s license, valid passport, military ID, or Naturalization
Certificate that readily identifies the holder. The parent(s) must submit supplemental
ID if driver’s license is out-of-state. Each parent or legal guardian must submit
photo identification for verification. Photocopies of the front and back of the ID
must be submitted with the application.
- One (1) Passport Photo, which may be obtained at the Passport Services office for an additional fee of $15.00.
U.S. Birth Certificate Requirement
Since April 1, 2011, the U.S. Department of State requires the full names of the
applicant’s parent(s) to be listed on all certified birth certificates to be considered
as primary evidence of U.S. citizenship and for parental identity for all passport
applicants, regardless of age. Certified birth certificates missing this information
will not be acceptable as evidence of citizenship.
Certain U.S. Passport applicants may be eligible to renew their U.S. Passport by mail using .
Applicants who do not meet Form DS-82 criteria are required to submit in person.

Should you renew your Passport by mail?
- You can submit the most recent full validity U.S. Passport book and/or U.S. Passport Card with the application and the passport is not damaged or mutilated.
- You were at least 16 years old when the most recent U.S. passport Book and/or U.S. Passport Card was issued.
- The most recent U.S. Passport Book and/or U.S. Passport Card was issued less than 15 years ago.
- You use the same name as on the most recent U.S. Passport Book and/or Passport Card OR you have had your name changed by marriage or court order and can submit proper original documentation to reflect the name change.
- Applicants who will renew by mail will complete Form DS-82.
Should you renew your Passport in person using Form DS-11? Yes, if:
- You were under the age of 16 when your most recent U.S. Passport was issued
- The most recent U.S. Passport is lost, stolen, damaged, or mutilated
- The most recent U.S. Passport expired more than 5 years ago and/or was not a full validity passport.
- You are using a different name than is listed on the most recent U.S. Passport and do not have proper documentation to reflect the name change.
Children under the age of 16 must renew their passports in person at a passport acceptance
facility using a completed Form DS-11.
An original or certified copy of the child’s birth certificate is required as well.
Required documents for renewal of a Passport by mail
- Completed Form DS-82, signed and dated.
- Original undamaged U.S. Passport meeting the above requirements.
- Certified copy of your marriage certificate or court order if your name has changed.
- Fees (check or money order only).
- A recent color photograph. ÃÛÌÒÓ°Ïñ Passport Services can assist with these for $15.00.
Note: In order to protect your application during mailing and processing, DO NOT FOLD the application. Use a mailing method that can be tracked through the United States Postal Service. The ÃÛÌÒÓ°Ïñ Campus Mail Center can provide free Priority Mail and Priority Express Mail envelopes. You may then take your completed application and envelope to the United States Post Office ( 755 Fairbanks St, Fairbanks, AK 99709) off of Geist Road to purchase postage and send it out.

(Passport fees are set by the U.S. Department of State and are the same at all passport application acceptance facilities.)
Passport Types
- Passport Book - may be used by U.S. citizens to travel by any means to any international travel destination.
- Passport Card - may be used by U.S. citizens to re-enter the United States at land border-crossings and sea ports-of-entry when arriving from Mexico, Canada, the Caribbean and Bermuda only.
- Note: Passport Cards CANNOT be used to fly internationally.

* Execution Fee - required in addition to the passport fee for each application. At ÃÛÌÒÓ°Ïñ Campus Mail Center Passport Services this fee can be paid by check, money order, debit/credit card or cash. Checks and money orders must be made payable to ÃÛÌÒÓ°Ïñ Campus Mail Center.
Passport Photo Fee
Passport photos are taken at the ÃÛÌÒÓ°Ïñ Campus Mail Center Passport Services office in the Facilities Services building at 803 Alumni Drive. The fee is $15.00.
The ÃÛÌÒÓ°Ïñ Campus Mail Center Passport Services office can provide photo services for your passport, visa, and other application needs.
Photos are $15.00 (payable by check, money order, credit/debit card or cash). If you require photos for another country’s passport or visa, please speak with one of our agents to determine if we can support the necessary photo measurements.
A recent color photograph is required with each U.S. Passport application. Passport photos must be:
- Color
- Clear, with a full-frontal view of the applicant’s face
- Printed on quality photo paper with a plain light (white or off-white) background
- Taken in normal street attire, without a hat, head covering, or dark glasses. Any head covering must not be worn unless a signed statement is submitted by the applicant verifying the item is used daily for religious purposes or a doctor’s statement is submitted verifying the item is used daily for medical purposes.
- Uniforms, clothing that looks like a uniform, and camouflage attire should not be worn in photos except in the case of religious attire that is worn daily.
- Taken within the last six months.
- 2 inches by 2 inches in size.
The image size measured from the bottom of your chin to the top of your head (including hair) should not be less than 1 inch and not more than 1 3/8 inches.
Passport forms may be filled in online and printed or the forms may be printed and
completed by hand
